The Urgent Response Services (URS) program was designed to be available for all children and youth registered in the OAP who meet defined criteria established by the ministry.

As part of the discharge process, we invite children/youth and families to complete this brief survey regarding your involvement with the Urgent Response Services program. The survey is 100% anonymous.
