About the Support Provider Database
The Support Provider Database facilitates the connection between families of children and youth with disabilities and Direct Support Providers in the Sudbury & Manitoulin region.
A Direct Support Providers is an independent respite provider who has been screened and made available to families through the Support Provider Database. Respite providers may come from a variety of backgrounds, from homemakers and students, to persons with training in nursing, education, child care, recreation, or rehabilitation. The screening process includes, but is not limited to, a screening interview, a Vulnerable Sector Check, the verification of references, and an orientation session.
The Respite Resource Coordinator is able to provide profiles (similar to a resume) of registered Direct Support Providers who are advertising their services through the database. You do not need to be receiving specific funding to use this service.
It is the responsibility of the parent/guardian to interview, select, negotiate wages, hours, and support details with the Direct Support Provider. The parent/guardian is also responsible to oversee that the service provided meets their expectations.
Direct Support Providers are not employees of the Children’s Community Network. Payment for respite services are paid directly by the family.
Characterizing Your Relationship With a Provider
It is the responsibility of the family and the respite provider to determine if they are entering an employer/employee relationship or a contracted service arrangement.

If you are interested in accessing profiles of pre-screened respite providers in the Sudbury & Manitoulin region, please complete the Family Registration Package below and submit it to respite@ccnsudbury.on.ca.
Your completed registration will be received by the Respite Resource Coordinator and you will be contacted.
